Attestation and Contract Tracing Form

We will be returning to the US and the Southwest Website says that we need to provide both the Attestation form and Contract Tracing form through Southwest.  It doesn't say how this information is provided to Southwest or where to access them. Are these paper forms?  Will we receive an email from Southwest before our return flight?  

The Southwest website does have a link to a general CDC page but it does not include the attestation form that I could see.

The attestation is done online when you check in. You simply click that you have read it. I have not heard anything contact tracing.
