To Phoenix then to Hawaii. I am vaccinated and am enrolled in the Hawaii Safe Travels program.
I just want to be fully prepared for everything that I will encounter at the airport and don’t want to have to cancel after I have already arrived.

You'd think I would have screen capped the declaration by now, so I'm just going by memory, but you are basically agreeing that you do not have any symptoms or feel unwell to travel, and you acknowledge that masks are required.
