Has anyone earned a companion pass without having first earned A List Preferred? That seems to be our case but does seem unusual to us and it has never happened in this order previously.

I woukd imagine it woud be easy to do that.
Get a 60,000 point bonus from Chase, and then spend a lot of money using the card. Couple that with maybe 20 anytime or business select flights, and I coud imagine someone easily getting to 110,00 points.

This is definitely rather common because, as @dfwskier mentioned, there are many ways to get points for CP while only points or segments associated with actually flying count for A List!
The first year we had CP, I was only A List. And even though I am now A List Preferred, the only reason we have CP is because of credit card points.
If you have the appetite for it, credit cards can be a valuable tool for travel with SW!
