I had a flight booked and due to chemo treatments I could not fly per doctor's orders--so I just accepted voucher credit but I just realized that the credit expires in July and I have'nt been cleared to fly until at least August---when I talked to customer service he said there is a form to complete to send in to request a refund--I can not find the form anywhere does anyone know where to find the form ?
