If packed in carry-on, they must be less than 4 inches from the pivot point.

The carry on policy is not being enforced.  I've seen on each and every flight those whose carry on is larger than the maximum allowed.  Even the basic eye can tell.

 

Also, carry on two large roller bags, one other bag plus a shopping bag.  Stop these people.
