When traveling as a designated "companion," is it standard SWA policy NOT to show the itinerary under the "My Account / My Trips" tab? Yes, one can view the itinerary after searching using first & last name plus record locator; but, why would it not show up automatically?

No, that's not usually how it works, sounds like your Rapid Rewards account number is not associated with the reservation. Verify the RR number the companion pass holder has listed for you is correct and then add or change the RR number on the existing reservation.
