If I have a voucher number/conf code, is there a way to see on what flight it was used?

assuming you kept your flight purchase confirmation e-mails, you can search your e-mail for the voucher number - it would be included in these e-mails.

I am unaware of any other way.

You would have received an email from Southwest which would have given you the information, but feel free to call Southwest as to the origin of the flight.  I keep track of mine on a spreadsheet with the source document related to the LUV voucher itself.

I had an agent tell me once which flight I used a voucher on but once that is booked then cancelled they can’t tell much.
