Visa card and health insurance

Just to confirm, when I purchase a ticket to Havana from TPA or FLL the medical insurance is included in the fare.  Also, how much is the visa card and is that purchased at the airport or does that need to be purchased in advance.  If in advance, is there a alink for that service?  Thanks.

I purchased our tourist cards thru the Southwest link and they cost $50.00 each.  If you have booked your flight, Southwest will send you additional information in an email.
