I don't recall though, did this promotion have a specific time period for companion pass? I recall some promotions the companion pass was only good for specific months. If this is the case you may not see companion pass until that time period, I have not had experience with this situation so someone else may be able to advise you better.
There is also usually a time limit to earn the $4k spending which might be three months? So earning it cumulatively in either one, two, or three months for example.
Usually you would get a confirmation email "you've made it" or something to that effect, but the pass itself won't show up until the specified period. If that was immediately then it would be there to nominate a companion, if it was July for example the pass won't show up in your account until 7/1 at midnight:01. This can be a little inconvenient if you are flying later in the day 7/1 but book your ticket now and add the companion then.

This normally works with A-list so I don't know why it didn't work for EBCI which should also be assigned at T-36.
I don't agree that you would have bumped up due to someone canceling - once you got your assignment at T-36 that should have been it - someone lucky checking in at T-24 would have gotten the abandoned position.
I did have an issue when I called to have the companion pass added vs when I had self-serviced it online. The clue was that although the companion can sign in and see their ticket, the companion ticket doesn't show up on the A-listers ticket so the association gets split somehow. If you ever notice that the companion isn't showing on the CP holder's ticket, assume you won't get sequential boarding.

When you get to talk to someone, or if you are automatically re-booked then you'll have to contact Southwest Contact Southwest Customer Service and let them know that you want to be refunded instead of rebooked, and if you are automatically rebooked let Southwest be the one to cancel it and refund, don't cancel it yourself if the only option is Travel Credit.
If you are starting from SFO consider trying OAK which has a lot of routes including to Denver with 5 or 6 non-stops per day.
SFO should still have 3-4 per day though - and if you are there at 2 a.m. now just take the 5 a.m. option first thing, obviously I'm reading this late but I hope you were able to get on that flight after all.

I suppose it is an informal policy - all you will find written is "any open seat" but never defining what an open seat is which we'll surely see some comments on this board in the follow ups that it can only mean a person in a seat and others that consider a saved seat next to someone not to be available.
Personally I'm all for someone saving a seat next to themselves for a spouse or child, up to a three-row section. People saving multiple rows, for co-workers, etc. no.
But basically it doesn't affect me that much and I just move on. I'm a happy guy and I'd like to stay that way and not let it bother me if I can help it.

Are you flying as a family very often? I would say it is atypical for a six year old to have a car seat onboard the airplane, but that's up to you as long as it is an FAA approved car seat.
The policy is too vague for this situation to be covered - could it be one adult and one older kid go with one younger kid and then the other adult and older kid go with the other younger kid? Maybe yes. The policy says "adult" but the older kids have adult tickets so do they count as adults in this context?
If that didn't work then have one adult and one older kid go with the two younger kids, and have the other adult and other older kid either come later in their position and save spots for them, or else get priority boarding for those two and have them get spots for all of you following later at family boarding.
It will be easy to save some spots together the further back you go in the plane if necessary.

That would be enough for an airport like HOU where it's almost all Southwest flights, you'll easily be able to get back to the domestic gates.
I've forgotten if there is a dedicated security screening for re-entry to the airport or if you go back to the main TSA checkpoint.

Summarizing from other posts the points can be restored sometimes if they can be intercepted based on whatever the hackers are doing with them, but many cases the points are spent in a way that Southwest can't verify what has happened. (You are allowed to book someone else a flight with your points for instance, so having only the example of another person booked with your points isn't evidence of fraud all by itself.)
There seems to be a mixture of examples where the points are or aren't able to be restored.

Most people are getting it at the ticket counter, you can put the request on your ticket when you buy it that you need wheelchair or other assistance, and if the passenger didn't do that then the ticket counter will also add it if you ask for a wheelchair then.
The pushers at the airport are looking for you to have registered your request at the ticket counter although sometimes you can get assistance without requesting it in advance if they aren't super busy, on many days it is a line of people waiting for someone.
The only passengers who might be slipping through en masse would be people using preboard who are ambulatory but have a hidden disability or require a certain seat. These passengers could be requesting preboard at the ticket counter or else at the gate.
Regarding your other comment that you always "buy an extra seat" do you mean Customer of Size? Then that program includes preboarding to ensure that you get access to two seats together, and then your companion would be included so this is normal for the policy, but since you aren't using wheelchair assistance it is fine for you to check in at the Gate rather than the ticket counter.

When you get your monthly statement, does the statement show that points are going to two RR accounts?
Since Southwest adds points to accounts at Chase's direction, the only way to fix this is to contact Chase.
Just to clarify @dmspen - the credit card points should go into only one of your rapid rewards accounts, and then you each should have separate RR accounts that will show your own points from flights (hotels, etc.)
So for instance your wife may get all the CC points and then see her own flight points, and then you may login and see only your own flight points. (or vice versa)
See if that's the case. You shouldn't both see the CC points from the same credit card on your individual RR accounts.

I don't know if that would work for the companions, that may be an area of improvement enforcing only one helper/companion per preboarding passenger. I'm not sure about actually marking the peoples' boarding passed with that designation though since they aren't able to use it without the person who has requested it. If the original preboard passenger didn't fly for some reason or changed flights the helper would still have it on their boarding pass - I'd say no to that, or it would have to be "preboard companion" or some other marking than the same "PRBD" marker.
