Traveling Post Surgery with Possible Person of Size

I am traveling to Mexico with my teenage son (5'8, 275lbs). He will be having major weight loss surgery.  He technically will be able to walk short distances, but shouldn't be walking or standing for long periods.  Should I book him 2 seats because of his size by putting XS on the 2nd ticket, or should I book him 2 seats because he's big and is temporarily disabled (put DXS on his ticket?).  I already anticipated buying the most expensive tickets both ways because I hate fighting over overhead bin space.

I'm not sure how the person-of-size reason codes work within a reservation but generally speaking...Yes, Southwest recommends purchasing the second seat in advance which helps ensure proper seating availability for everyone and helps avert an oversale situation in case the flight becomes full. Second seats requested at the last-minute at the airport could create an oversale situation aboard full flights. After your travel is finished, contact Customer Service and SW should issue you a refund for the second seat purchase.

Since your son also has a special need regarding his walking/standing needs, he and one other travel companion may also qualify to pre-board in lieu of utilizing a higher fare product for the priority boarding and overhead bin access near your seat. I would work with your manager, share your concerns and ask if there’s a way to provide an accommodation for you based on your medical conditions. I would do so over email and I would specifically mention “reasonable accommodation” to ensure they actually have to look into it rather than thinking it’s an administrative request/preference thing and saying no.
