Leaving from Sky Harbor/PHX to Maui

Has anyone left Sky Harbor/Phoenix recently for Maui?
We are seeking the vaxx exemption. Flying on Southwest. How long before our flight do we need to arrive at the airport?
We are both fully vaccinated. Would you also get tested as backup?

Also I had an issue completing the online application…. It would not accept my (typical) Phoenix address that sounded like Spanish (Camello Del Norte). It said I had to put it in English. After several attempts and a couple hours on hold, I made is North Camel CA and it went through but it won’t match my DL! Ideas?

The site says pre clearance ends 30 minutes before the departure time.
