How to replace a misplaced luv voucher

Hi All,

About 5 months ago I received two $100 LUV vouchers, one for my wife, one for me. Planning another trip and want to use them but can't seem to find them. My daughters family was on same flight and also received Luv vouchers and has since used them. How do I go about getting mine replaced?
