I got a new premeir card in Oct 2016 with $2000 being spent in the first 3 months getting 50,000 bonus points. I had a disputed charge made to my account in October 2017 that put me over the $2000 mark in month 2 (November) that was credited back to my account in November however the points were still awarded in November after the November statement was paid. The net payment gave me a credit balance on the card of $269.99 which was the amount of the disputed charge that was refunded since I paid the balance on the card on the due date in early November. I was trying to qualify for the companion pass in 2017 but since the 50,000 hit my account in 2016 I don't see how I can make it. When I read the terms on qualifying it said that if I hit the $2000 point and had funds credited that would take me back below the $2000 that the rewards points would not be awarded until the balance of the next statement was paid that took me over the $2000 point range. I made the purchases that would take me over the mark in December of 2016 but the December statement did not process until late December so it did not hit the account until 2017. The way that I am reading the terms and conditoins, I think the rewards points should be awarded in Jan of 2017.

A-List tier qualifying points are very limited. Essentially, they are only those earned through actual flying ("butt in seat"), plus the small amount earned specifically by the Premier Southwest Visa card. "A Member who has a Rapid Rewards Premier Card from Chase will earn 1,500 Tier Qualifying Points for every $10,000 in eligible net purchases on that card, up to 15,000 Tier Qualifying Points per year."

It does but if you have not already opened the card, you should try to make sure that your points will post in January, as if you earn the CP in 2017 it will be good for all of 2017 and 2018! If you have opened it, count the days from the time you first applied and see if you can time meeting the minimun spend in January, unless you already have a lot of points to go with the 40,000. The points all have to be earned in the same calendar year. If you needed them this year to go with points you already have, you had better make your minimum spend quickly, as the points have to post to your account before December 31st. Your points post a few days after the statement closes, not after your payment due date.

While this says "solved" part of your question seemed to involve why it didn't ask for your KTN, it just asked for your passport information. The reason is (for some reason) the system does not yet automatically add your KTN to an international flight, as it does for domestic flights. You have to call SW to have it added to each ticket. I just booked to Montego Bay a few weeks ago, and their was some type of little pop up that told what to do, when I was purchasing the tickets. Same thing when we went to Belize. I think the new system that is supposed to be up and running sometime soon will probably fix this.
