It will take about a month to receive the card. I have never needed a card to book or check in. Your companion benefit should be immediately available after the point posting that takes you over the 110K points. At that point you will see a spot on your online dashboard to enter your companion or call SWA and they can enter the details over the phone. After the companion is successfully added you can begin booking companion reservations. No waiting. It used to be a requirement that they have a Rapid Reward number, so go ahead and begin that process, if your companion does not have an RR #.

Redeposited, yes. Expiration is the usual points expiration policy, your account would need to be inactive for 24 months. No expiration date specific to the refunded points.

Standby isn't the same as rebooking. 😉 Experiences vary, likely because the situations vary as well. If there are many open seats, an agent may be willing to rebook both onto the new flight. If there aren't many open seats, the A-Lister is put on standby, and it remains unclear what priority is given to -- for example -- the last two remaining seats. Should another A-Lister be given the last seat, over an unpaid companion? If a seat is available, the companion gets it. But that may be only after all the standbys are cleared.
