Whether Chase is a business partner, an alien from another galaxy, a member of the 5th dimension is completely irrelevant.
You were on Southwest's web page and they allowed to content to be presented.
I would request that the inquiry be removed and your application be withdrawn. I would also seek compensation for the $200 credit not received.
There needs to be accountability on Southwest. You weren't buying from the Chase website.

Request a refund of the wifi cost. We see nonstop complaints about the wifi and on my last flight, there was an announcement by the flight attendant making somewhat of a joke about the spotty wifi service.
If Southwest wants to sell wifi, it needs to work. Otherwise, give refunds. End of story.

As a Southwest customer and shareholder, someone who stays within the size and weight limits of bags, I would prefer that Southwest refuse to load anything other than baggage. All of this slows down the boarding process and adds to the cost of doing business.
Southwest never agreed to be part of your exercise regiment and you can ship your bike though other channels if that is critical to your life. I would be in favor of Southwest stopping all items that are not baggage.
Refer all of those people to a freight shipper.

There is no mention of preboarding in the question. For each passenger that qualifies to preboard. one accompanying passenger may board with them.
What Southwest is doing is not "family boarding", it is better described as "child assist". I have called and asked the question several times and always get the same answer. That's the reason that I construct my response as it was described to me.

As a passenger, I arrive at the check-in and announce to the agent the following:
"I need to preboard and am self-identifying as an individual with a disability who needs to do so to be seated."
If you use the term "extra time" you will be designated between Group A and Group B boarding. So, use your words carefully.
Any further questioning inhibits a person's rights.

There is nothing vague about family boarding. For each child under the age of seven, up to two adults may board with that child.
For a 15 year old, family boarding does not apply. The only thing that I could consider would be if the 15 year old had their own child and then they would likely board with the child as an "adult".
I have researched this and found that while the above is correct, the gate agents don't always follow the rules.

The law requires Southwest to grant preboarding to those who make the request.
The "fact finding" is basically asking if you need some extra time and if you do, you will be assigned an "extra time" designation which occurs between A and B boarding.
Under the law, you are not required to respond to questions. You declare that you self identify as an individual with a disability and you either do or do not require assistance to board. That's all there is to it.
Any system has abuse. I've seen people at the wholesale club pitch a fit about a coupon that is expired and eventually get the coupon. I see people abusing the traffic signals every day. Yes, they abuse the preboard too. However, as I have stated before, the core issue is open seating. With assigned seating, there is no incentive to abuse the preboard system.

Contact Southwest and obtain a complete refund. In addition, file a complaint with the US Department of Transportation. Southwest needs to take people who save seats and have them removed from the plane.

What gets back to me is that I am one of the very silent people who occasionally calls and complains about policies. I am hearing that people are demanding partial refunds and filing disputes on their credit cards for partial rebates of fees paid. No one wants to follow the rules and then watch others break those rules when the employee stands there and does nothing.
Disney has rules, Alamo Rental has rules and so does Southwest. It is beyond time for these gate agents to do their jobs.

If you see nonsense, call the Southwest Customer Service lines. I previously had a long conversation and a call back from a Senior Manager. The days of the whole family, aunt-uncle-cousin-grandma-grandpa-mom-dad-nanny-adult children abusing the family boarding are OVER.
Also gone are the attempts to fill the airplane with three, four and five carry-on items as well as trying to bring things on the plane that are WAY over the size limit.

As a C-suite executive myself, in another industry, I can tell you that this issue if not top priority. No one, and I mean NO ONE, is going to take an adversarial position against persons with disabilities. The primary policy of Southwest is OPEN SEATING which is the source of the entire discussion.
Remove the OPEN SEATING, and the problem disappears.
Southwest is NOT going to go to assigned seating for A-list passengers and open seating for the remainder. It is simply NOT going to happen.
You have to decide which is your primary concern - cost of seating position.

My last experience with Southwest was positive. I found that the gate agents at MCO are enforcing the former runaway practices of family boarding, oversized luggage and number of pieces.
I saw examples of strict enforcement of all three and I applaud Southwest for getting a handle on this. I saw a case where a 13 year old was denied family boarding since they didn't fit the definition. I saw a case where the luggage was oversized and they had to check the bag. I saw a case where someone tried to sneak a small third item and they were told NO.
MCO has been lax in enforcing these policies for too long. Based upon the conversation that I heard from other passengers, they're glad to see that there is now law and order.
I do not expect the steakhouse to invent a vegan menu for me and I do not expect Southwest to sell discounted fares, and then come up with specific seating assignments and other first class accommodations.

I believe that Southwest's pricing and policies are VERY REASONABLE.
The bicycle takes up a lot of space and is more difficult to handle than a piece of luggage. Thus, the policy on weight and size limitations.
The choice is simple - pay to travel with the bicycle or travel via other means.

I would propose the following announcement before each flight:
As a reminder to all passengers, Southwest has a policy of open seating and all unoccupied seats during boarding are available to you. Should another passenger limit your choice of seats by claiming that they are saved or otherwise dissuade you from making a seat selection, the individual limiting your choice will be asked to exit the airplane and their ticket will be cancelled without a refund or credit. We want everyone to enjoy flying Southwest and respect the open seating policy.
